People 65+ represented 14.1% of the population in the year 2013 but are expected to grow to be 21.7% of the population by 2040.”

A geriatric social worker and/or case manager is a professional with expertise working with adults age 65 and older.  Such professionals have experience working in hospital, nursing home, and community settings. As a result of having experience across this continuum of care, geriatric social workers are often savvy navigators of the complicated healthcare and social service systems.  They can be a tremendous support to you and your family during a time when you are overwhelmed yet trying to do what’s best for your loved one.

Geriatric social workers/case managers complete professional assessments that determine the needs of an individual and then develop an appropriate care plan to meet those needs. They are often responsible for implementing care plans while collaborating with other healthcare professionals. Additionally, they are knowledgeable regarding financial resources often needed to support one’s care plan.

Once a care plan is implemented it is the geriatric social worker/case manager’s responsibility to monitor the services and the care plan. They will reassess the needs and care plan regularly and recommend adjustments accordingly. Many find geriatric case management is especially helpful for those living far from their elderly loved one. Peace of mind is knowing that their loved one’s progress at home is being monitored when they cannot monitor themselves.
